Intake Form

This agreement ("Agreement") is between Angel's Pet Boarding, LLC ("Angel's") and you, the pet owner ("Owner").

1. Pet Information and Vaccination:​

   • Angel's must be made aware of any pets that are not Spayed/Neutered.

   • Vaccinations: Owner confirms their dog(s) have current vaccinations for rabies, distemper, and Bordetella. Owner understands that even with vaccinations, their dog(s) can still contract illnesses.

   • Flea prevention: Owner confirms that the pet is currently on flea prevention medication.

2. Photo and Video Release:

   • Owner allows Angel's to use their pet's name and image for marketing purposes without compensation.

3. Group Play:

   • All dogs must pass a temperament assessment.

   • Aggressive dogs cannot participate in group play.

   • By admitting their dog(s) into group play, Owner confirms their dog(s) are healthy, haven't harmed other animals, and haven't shown aggression.

   • Angel's staff can remove any dog demonstrating unacceptable behavior.

4. Release of Liability:

   • Angel's will take reasonable care to prevent injuries, but minor injuries can occur during play.

   • Owner releases Angel's from liability for illness, injury, death, or escape, provided reasonable care is taken.

5. Personal Property:

   • Owner is solely responsible for any harm their pet(s) cause to others or property.

   • Angel's is not responsible for lost or damaged personal belongings.

6. Food and Medication:

   • Owner is responsible for providing enough food and medication for their pet(s)' stay.

   • If food/medication needs replacement, Angel's can purchase it, and Owner agrees to reimburse the cost plus a $25 fee.

7. Veterinary Care:

   • Angel's can authorize medical treatment for injured or ill pets if the option on this agreement is selected below.

   • Owner is responsible for all treatment and transportation costs.

   • Angel's may use Owner's vet or the nearest 24-hour vet hospital.

8. Additional Policies:

   • Owners of special-needs pets, young puppies/kittens, or senior pets acknowledge the inherent risks involved and waive claims for illness or injury.

   • Angel's will transport deceased elderly pets to Owner's vet (within 15 miles) or the nearest 24-hour vet if closed. Transportation fees apply.

   • Grooming fees may apply if a pet's nails require trimming or if they soil themselves.

   • Payments are due at the end of each visit.

   • Cancellations require 24-hour notice, or a one-night stay fee applies.

   • Angel's reserves the right to refuse admittance or dismiss any pet that doesn't meet their standards.

9. Agreement Acceptance:

   • By signing this Agreement, Owner acknowledges reading and understanding all terms and conditions and agrees to be bound by them.
